Privacy Policy
Last updated: October 18, 2026
NewLastName.org ("we," "us," or "our") respects your privacy. This Privacy Policy explains what information we collect, how we use it, and your rights regarding your data.
1. Information We Collect
1.1 Information You Provide
When you use our form-filling tools, you may provide:
- Standard personal information: Full legal name (current and new), date of birth, address, phone number, email address, spouse name, place of birth, parent names
- Name change details: State, county, name change type, reason for change
- Email address: If you sign up for launch notifications or create an account
1.2 Social Security, driver’s license and passport numbers
When you create a filled form, what you type (which can include your Social Security number) is sent to our server to build the PDF. Saved drafts don't keep your SSN, driver's license or passport numbers. Only enter your SSN if you need it on the form.
1.3 Automatically Collected Information
We may collect standard web analytics data including:
- IP address (Google uses it to estimate approximate location; Google Analytics does not store it)
- Browser type and version
- Pages visited and time spent
- Referring URL
Analytics. We use Google Analytics 4 to count visits and to understand how visitors use our site, such as which pages are read and how people arrive. In the United States and most other countries it is on by default; you don’t have to click anything. Google Analytics sets cookies (named _ga and _ga_ followed by our measurement ID) and collects information such as pages viewed, approximate location (city level), device and browser type, and referring site. We do not send Google your name, email address, or anything you type into our tools or forms, and we do not load Google Analytics on our form tools. Google signals and ad personalization are turned off in our analytics, so analytics data is not used to show you ads. Google processes this data under its own policies: How Google uses information from sites that use its services.
Advertising. We show ads served by Google AdSense. Google uses cookies (such as __gads, __gpi and __eoi) to serve ads, limit how often you see the same ad, and measure ads. We ask Google for restricted data processing and set ad personalization to “denied,” so Google does not use information from this site to personalize ads for you. Learn more: How Google uses cookies in advertising.
Visitors in the EEA, UK and Switzerland. If you visit from the European Economic Area, the United Kingdom or Switzerland, analytics cookies are off by default, and Google applies its own EU consent rules to ads. Unless you consent, Google Analytics does not set or read analytics cookies on your device, though Google may still receive basic signals without cookies, such as that a page was viewed. Google AdSense may still set advertising cookies; to stop them, use Your choices below or turn on Global Privacy Control.
Your choices: Do Not Sell or Share / Opt out of analytics
- Opt out on this site: use the button below. We save your choice in this browser, and from then on we do not load Google Analytics and advertising cookies are turned off on this browser. If you clear your browser data, or use another browser or device, opt out there too.
- Global Privacy Control (GPC): we honor it. If your browser sends a GPC signal, we treat it as a request to opt out of the sale or sharing of your personal information: we do not load Google Analytics, and advertising cookies are turned off, for that browser.
- Google’s tools: Google’s opt-out add-on blocks Google Analytics on every site, and Google My Ad Center controls Google ads. You can also block cookies in your browser settings.
Google Analytics is on for this browser.
Do Not Track. Our site does not currently respond to browser Do Not Track signals, because there is no common standard for them. We do honor Global Privacy Control, as described above. Google, as our analytics and advertising provider, may collect information about your online activity over time and across different websites.
Retention. Analytics data is kept for 2 months.
2. How We Use Your Information
We use your information solely to:
- Run the optional form tool on your device on your behalf
- Save your progress so you can return and complete your forms
- Send you launch notifications (if you opted in)
- Authenticate your account (magic link login via email)
- Improve the Service through aggregated, anonymized analytics
We do not sell, rent, or share your personal information with third parties for marketing purposes. Ever. Google’s analytics and advertising cookies are described in section 1.3, and you can opt out of them there.
3. How We Store Your Information
3.1 Form Data
Your form data (names, addresses, dates) is stored in an encrypted database while your session is active. This allows you to save progress and return later. Form data is encrypted at rest using industry-standard encryption.
3.2 Generated PDFs
Completed PDF forms are generated on our server and made available for download.
3.3 Session Data
We use anonymous session tokens (UUIDs) to associate your form data with your browser session. No login is required for basic use. Sessions that have been inactive for more than 30 days are automatically deleted.
4. Third-Party Services
We use the following third-party services:
- Resend (resend.com) — transactional email delivery for account authentication and launch notifications
- Google Analytics (Google LLC): website usage statistics, under Google’s terms.
- Google AdSense (Google LLC): the ads shown on our pages, under Google’s terms (see “Advertising” in section 1.3).
- AI providers (for form field detection only) — when you upload a custom PDF, we may use AI services to detect form field names. Only field names and structure are sent, never your personal data.
5. Cookies
Answers you enter before signing in are saved only in your browser on this device, deleted after 7 days of inactivity or when you clear them or sign in. Use a private window on a shared device.
We use minimal cookies:
- Session cookie: keeps you signed in after you log in, so your saved answers and forms stay with your account.
- Analytics cookies: Google Analytics cookies for traffic data (see “Analytics” in section 1.3)
- Advertising cookies: Google AdSense cookies (see “Advertising” in section 1.3).
To turn off analytics and advertising cookies, see Your choices.
6. Your Rights
You have the right to:
- Access: Request a copy of the personal data we hold about you
- Delete: Request deletion of your data at any time by emailing us
- Correct: Update or correct your information through the app or by contacting us
- Opt out: Unsubscribe from marketing emails at any time

California Residents (CCPA)
If you are a California resident, you have additional rights under the California Consumer Privacy Act:
- Right to know what personal information is collected
- Right to delete personal information
- Right to opt out of the sale or sharing of personal information. We do not sell your data. To opt out of Google’s analytics and advertising cookies, use the “Do Not Sell or Share / Opt out of analytics” link in our footer (it leads to Your choices) or turn on Global Privacy Control in your browser.
- Right to non-discrimination for exercising your rights
7. Data Security
We implement security measures to protect your information:
- HTTPS/TLS encryption for all data in transit
- Application-layer encryption for stored form data
- No sensitive data in server logs or error messages
- Automatic data deletion after retention period
No system is 100% secure. While we strive to protect your information, we cannot guarantee absolute security.
8. Children's Privacy
The Service is not directed to children under 13. We do not knowingly collect information from children under 13. The minor name change feature is intended for use by parents or legal guardians filing on behalf of a child.
9. Data Retention
- Form sessions: Automatically deleted after 30 days of inactivity
- Account data: Retained until you request deletion
- Email subscriptions: Retained until you unsubscribe
10. Changes to This Policy
We may update this Privacy Policy from time to time. We will post changes on this page and update the "Last updated" date at the top. Continued use of the Service after changes constitutes acceptance of the updated policy.
